Required Forms

All forms must be filled out completely.  If not, they will be returned

and asked to complete and no spot will be reserved for your child. 
 

If FINANCIAL ASSISTANCE is needed

                     A confidential scholarship fund is available to help subsidize membership and/or program fees

                        for those who demonstrate financial need.  The ability of the YMCA to respond to such requests

                         is dependent on the success of fund raising projects including the Annual Support Campaign,

                          operating finances and specific program costs.  All applications must include two proofs of income.
